Reporting to the Residential Admissions Manager, the Residential Admissions Specialist will work collaboratively with stakeholders across our continuum of care. They will assess the care needs of new consumers entering Montefiore in a meaningful way ensuring a positive customer experience.Primarily, you will have responsibility for undertaking pre assessments of potential consumers of Montefiore and recommending which of our core services best meets their individual needs (including residential care, independent living and home care).This role is an essential part of the Consumer Journey, as one of the first points of contact for our residential aged care clients. The Residential Admissions Specialist will be a vital ambassador of our valued and respected organisation.The role is integral in providing high quality and accurate clinical assessments combined with outstanding customer service to ensure the admissions process is professional, accurate and robust.Specifically, the key capabilities will include, but are not limited to:Deliver the Montefiore Experience (ME) approach that supports both the resident and their familyAssess new consumers clinical/care requirementsUtilise knowledge of all Montefiore services to identify the most appropriate unit/service at Montefiore to care for the consumer’s individual needsDevelop and maintain assessment processes and relevant documentationRespect and advocate for the rights of consumers at MontefioreDevelop strong relationships with line managers in each unitEssential Criteria:Nursing or Social Work/Counselling qualifications.5 years minimum professional experience as a social worker or clinical worker.Aged Care ExperienceCompetency in clinical documentation, administration, and IT SystemsStrong customer service focusOutstanding level of professionalism with excellent grooming standardsEffective communication skills, both oral and writtenDemonstrated experience in multi-tasking with the ability to meet competing deadlinesAbility to maintain customer confidentiality and privacyHigh levels of integrity, empathy and compassionDriver’s licence and own vehicle and willingness to travel to conduct pre-accommodation visitsWe would love to hear from you if you are compassionate, enjoy liaising with people and providing an outstanding customer journey.  About Us:Montefiore is an award-winning, Sydney-based aged care provider, with a proud 130-year history as a non-profit organisation founded on Jewish values.
