About Help at Home

Our Care Coordinators can help you to choose from the range of services provided by Help at Home, so you can continue to live independently with confidence.

 

Help at Home is currently available to clients in the Eastern Suburbs, Northern Beaches, Upper and Lower North Shore, Hunters Hill, Ryde, North West and Hills District, Mosman and surrounds, and the Inner West.

 

You can access Help at Home though:

 

  • Home Care Packages (HCP) – A government subsidy for older people
  • National Disability Insurance Scheme (NDIS) – For people of all ages living in the community with a disability
  • Fee for Service – For those waiting to have subsidised care approved or who don’t qualify for a Home Care Package or the NDIS.

Whatever your budget or independent care needs, we will provide your own Care Coordinator to put together an individually tailored program.

Respite care

Health and wellbeing is as important for carers and families as it is for the people they care for. We provide our range of exceptional respite options, delivering the high-quality care that Montefiore is known for, in your own home.

Domestic assistance

We are here to lend a hand with everyday tasks, such as cooking, cleaning, gardening and shopping, as well as friendship and companionship.

Occupational therapy

To enable you to participate as fully as possible in the activities that are meaningful to you, our expert Occupational Therapists will work with you to support you with age-related conditions such as: balance or coordination issues; cognitive decline, dementia or memory loss; and vision or hearing impairment.

24/7 clinical and nursing care

Our 1800 Help at Home hotline is staffed by clinical healthcare professionals 24/7, who are ready to discuss the type of care you want or book an appointment for you.

Allied health

Montefiore has more than 120+ allied health professionals on staff, helping manage the specific health challenges associated with ageing and improve quality of life.

Dementia/COPE

Our dementia expertise and specialised programs are designed to support people living with dementia and their carers. Montefiore has also partnered with the University of Sydney and Flinders University, to deliver COPE (Care of People with Dementia in their Environments), a program designed to support and empower people with dementia and their carers. This practical program is delivered by Montefiore occupational therapists and nurses and includes a comprehensive assessment, support for carer wellbeing and tailored activities and strategies to promote independence and overcome challenges.

Personal care

Your personal Care Coordinator will design and manage a program tailored to your needs and budget, delivered by a trusted team of regular carers. Services may include medication administration, shower assistance, bowel and bladder care, wound management and palliative care.

Transport

Our trusted carers are able to facilitate and provide transport services, which may include attending medical appointments or visiting shops.

Companionship

Private companions are available to visit clients providing social engagement and assistance with outings when loved ones are unable to be there.  These services are also available to clients living in Residential Care.

HCP Management

Home Care Packages (HCP) can either be self-managed or provider managed. You can find information below on the difference between the two.

How to Apply

You can self refer for an evaluation by an Aged Care Assessment Team (ACAT) at myagedcare.gov.au or obtain a referral from your GP. Our Help at Home team is always happy to assist you as one of the expert home care package providers. Once your CDC has been approved, our Care Coordinators will help you work out the best services for your package level and care needs.

Once you have funding approved, we can assist by offering services such as support coordination, daily living support, social and community participation, therapy services and plan management.

If you don’t qualify for CDC or NDIS subsidies or are waiting for approval, Help at Home also offers private, fee for service care. Call our hotline for a verbal quote or to arrange an obligation-free consultation at home.

Frequently asked questions

Help at Home is an in-home support service provided by Montefiore to care recipients. Services are based on individual care plans that offer as little or as much support as you require.

Help at Home offers extensive support services, including transport, meal preparation, companionship, shopping, personal care, nursing and clinical care, dementia support, occupational therapy and physiotherapy, as well as 24/7 support for recipients who have high care needs.

Our dedicated Care Coordinators manage your individual care needs. They are your main point of contact regarding your circumstances and any changes to your care needs or government package. They are also available for general questions you may have regarding your care.

Cost varies depending on whether you are a self-managed retiree or have a Home Care Package. If you have means and the government has not provided you with a package, the approximate cost for support within your home is $55 per hour.

If you are receiving a Home Care Package from the government, the approximate cost is $66 per hour.

Nursing costs vary depending on the services required.

Yes, our team will assist and manage your introduction to the Montefiore Admissions Team and ensure you feel supported and informed when making decisions about the care required for you or a loved one.

Our care staff can provide additional support, companionship or transport services if required when living in residential care.
