How Does It Work?
Help at Home by Montefiore’s disability support services are available to NDIS participants who are Self-Managed, Plan-Managed, and Agency Managed. Whatever your budget or care needs, we can help you put together an individually tailored program.
Our NDIS Support Coordinators will assist you with understanding your NDIS plan and recommending providers, services, and supports. This includes connecting you with mainstream services and supporting your full participation in the community. Our Coordinators can also assist you in preparing for unforeseen circumstances, including access to crisis services.
