Step-by-step guide
This step-by-step guide will help you to understand the application process for residential care, and ensure we have all the information we need.

Before you apply for permanent or respite residential aged care, you need a face-to-face Aged Care Assessment by an Aged Care Assessment Team (ACAT).
You can book online at myagedcare.gov.au, by phone on 1800 200 422 or via your GP.
When your assessment is complete, provide us with your referral code and support plan.

The form requests all the key information required for Montefiore to plan the right care for your needs, as well as understand your accommodation preferences.

Ask the GP who is most familiar with your medical history to complete the Medical Evaluation form below.
Everyone has different care and medical needs, and the information from your GP together with the ACAT report will allow us to help tailor the right care and support for you.

Return all the items from steps 1-3, along with copies of any applicable Power of Attorney and/or Guardianship documents, to our Admissions team via:

Once your paperwork has been submitted, a member of our team will arrange a personalised tour of the Residential Care campus that best suits your needs.
We will also assist you with the next steps in the application process, including providing you with the appropriate Australian Government Aged Care Income or Means Assessment form.

The Government Aged Care Income or Means (income & assets) Assessment is needed to determine your permanent daily fees – learn more at myagedcare.gov.au/income-and-means-assessments.
Sign the completed form in front of a JP or solicitor and lodge it. (If you have previously completed this step, please provide the fee letter.)

A member of our Admissions team will meet with you in person to assess which of our care units would be best for you to live in, based on your individual care needs.

We will contact you to offer you a place at Montefiore Residential Care as soon as we have matched you with an available room.
If you accept, we will issue a Residential Care Agreement and make a time for you to come and sign it (along with other Admission paperwork).
